=== Hepatic encephalopathy === Hepatic encephalopathy is a potential complication of cirrhosis. It may lead to functional neurological impairment ranging from mild confusion to coma. Hepatic encephalopathy is primarily caused by the accumulation of ammonia in the blood, which causes neurotoxicity when crossing the blood-brain barrier. Ammonia is normally metabolized by the liver; as cirrhosis causes both decreased liver function and increased portosystemic shunting (allowing blood to bypass the liver), systemic ammonia levels gradually rise and lead to encephalopathy. Most pharmaceutical approaches to treating hepatic encephalopathy focus on reducing ammonia levels. Per 2014 guidelines, the first-line treatment involves the use of lactulose, a non-absorbable disaccharide which decreases the pH level of the colon when it is metabolized by intestinal bacteria. The lower colonic pH causes increased conversion of ammonia into ammonium, which is then excreted from the body. Rifaximin, an antibiotic that inhibits the function of ammonia-producing bacteria in the gastrointestinal tract, is recommended for use in combination with lactulose as prophylaxis against recurrent episodes of hepatic encephalopathy. In addition to pharmacotherapy, providing proper hydration and nutritional support is also essential. Appropriate quantities of protein uptake are encouraged. == Research == Progesterone was studied as a progestogen-only injectable contraceptive, but was never marketed. Combinations of estradiol and progesterone as a macrocrystalline aqueous suspension and as an aqueous suspension of microspheres have been studied as once-a-month combined injectable contraceptives, but were likewise never marketed. Progesterone has been assessed for the suppression of sex drive and spermatogenesis in men. In one study, 100 mg rectal suppositories of progesterone given five times per day for 9 days resulted in progesterone levels of 5.5 to 29 ng/mL and suppressed circulating testosterone and growth hormone levels by about 50% in men, but did not affect libido or erectile potency in this short treatment period. In other studies, 50 mg/day progesterone by intramuscular injection for 10 weeks in men produced azoospermia, decreased testicular size, markedly suppressed libido and erectile potency, and resulted in minimal semen volume upon ejaculation. An oil and water nanoemulsion of progesterone (particles of <1 mm in diameter) using micellar nanoparticle technology for transdermal administration known as Progestsorb NE was under development by Novavax for use in menopausal hormone therapy in the 2000s. == Medical uses == Lorcaserin was used long term for weight loss in those who are obese. The safety and efficacy of Belviq were evaluated in three randomized, placebo-controlled trials that included nearly 8,000 obese and overweight patients, with and without type 2 diabetes, treated for 52 to 104 weeks. All participants received lifestyle modification that consisted of a reduced calorie diet and exercise counseling. Compared with placebo, treatment with Belviq for up to one year was associated with average weight loss ranging from 3 percent to 3.7 percent. About 47 percent of patients without type 2 diabetes lost at least 5 percent of their body weight compared with about 23 percent of patients treated with placebo. In people with type 2 diabetes, about 38 percent of patients treated with Belviq and 16 percent treated with placebo lost at least 5 percent of their body weight. Belviq treatment was associated with favorable changes in glycemic control in those with type 2 diabetes. The approved labeling for Belviq recommends that the drug be discontinued in patients who fail to lose 5 percent of their body weight after 12 weeks of treatment, as these patients are unlikely to achieve clinically meaningful weight loss with continued treatment. The drug's manufacturer was required to conduct six postmarketing studies, including a long-term cardiovascular outcomes trial to assess the effect of Belviq on the risk for major adverse cardiac events such as heart attack and stroke.
=== ATLAS and P-body === ATLAS and P-body (known as Blue and Orange to GLaDOS) are player characters in the co-op campaign of Portal 2. They are two robots that cannot speak and can only produce grunts, which are voiced by Dee Bradley Baker. They are equipped with their own portal guns which bears the colour of its user (blue and purple for ATLAS, yellow and red for P-body). Atlas and P-Body are described as masculine and feminine, respectively, as stated by Chet Faliszek in an interview. During the campaign, GLaDOS calls them Blue and Orange, respectively, and constantly tries to ruin their relationship by awarding one and ignoring the other. GLaDOS sends ATLAS and P-body into four areas to access data discs. Once done, they are blown up, as GLaDOS claims it is the only way they can be retrieved from these areas. Later, GLaDOS commands them to retrieve a certain "surprise" in an area. They go in search, only to find hundreds of thousands of other test subjects in suspended animation. After the co-op campaign is finished, GLaDOS quotes that they saved science. ATLAS and P-body begin to celebrate, only to be promptly blown up by GLaDOS. In the DLC campaign "Peer Review", ATLAS and P-body are reactivated by GLaDOS one week after the original co-op campaign, during which GLaDOS has already wiped out all of the found test subjects in her attempts to turn them into "killing machines". The two robots are sent to find a saboteur that has taken control of a prototype central core and is causing problems in the facility.
== Adverse effects == Genital infections seem to be the most common adverse effect of gliflozins. In clinical trials fungal infections, urinary tract infections and osmotic diuresis were higher in patients treated with gliflozins. In May 2015, the FDA issued a warning that gliflozins can increase risk of diabetic ketoacidosis (DKA, a serious condition in which the body produces high levels of blood acids called ketones). By reducing glucose blood circulation, gliflozins cause less stimulation of endogenous insulin secretion or lower dose of exogenous insulin that results in diabetic ketoacidosis. They can specifically cause euglycemic DKA (euDKA, DKA where the blood sugar is not elevated) because of the renal tubular absorption of ketone bodies. A particularly high risk period for ketoacidosis is the perioperative period. SGLT2 inhibitors may need to be discontinued before surgery, and are only recommended when someone is not unwell, is adequately hydrated, and can consume a regular diet. Symptoms of ketoacidosis include nausea, vomiting, abdominal pain, tiredness, and trouble breathing. To lessen the risk of developing ketoacidosis after surgery, the FDA has approved changes to the prescribing information for SGLT2 inhibitor diabetes medicines to recommend they be stopped temporarily before scheduled surgery. Canagliflozin, dapagliflozin, and empagliflozin should each be stopped at least three days before, and ertugliflozin should be stopped at least four days before scheduled surgery.
== Evo Morales == In 2008, President Evo Morales gave the Drug Enforcement Administration (DEA) three months to leave the country, accusing them of fomenting the drug trade rather than fighting it. President Morales continued to maintain relations with the US government, including on counter-narcotics issues. Such relations appeared to have been strengthened by the Morales administration's success in reducing coca cultivation. Its strategy was based on the voluntary participation of farmers from all coca-growing regions in the country. For instance, farmers in Chapare are allowed to grow one cato (1,600 square meters) of coca per year, as part of policy formally introduced in Bolivia in 2004. Any coca grown beyond that limit, or any cultivation outside of approved coca-cultivation regions such as Chapare, is subject to elimination. The strategy relies on coca growers federations’ ability to enforce the agreement. Such federations are influential, and penalties for violations by farmers or lax enforcement by federations can be stern (including seizure of lands). As a result, coca cultivation in Bolivia fell to 27,200 hectares in 2011 from 31,000 hectares in 2010 - a 12 percent decrease. Former President of Bolivia Evo Morales is also titular president of Bolivia's cocalero movement – a loose federation of coca growers' unions, made up of campesinos who are resisting the efforts of the United States government to eradicate coca in the province of Chapare in central Bolivia.
